<div>　山形民医連では、県内全病院（66か所）に経営状況に関するアンケートをお願いし、17病院から回答をいただきました（回収率25％）。<br>　回答には、県内病院の経営が非常に厳しいことが表れています。2024年度の診療報酬改定によって「増収増益になった」と回答した医療機関は、わずか1病院のみでした。「ベースアップ評価料や基本診療料の改定は、処遇改善の目標に対して不十分」とする回答は11病院で、6割を超えています。資金繰りについては10病院が「厳しい」、2病院が「やや厳しい」と回答。医療従事者の体制については「医師、看護師が不足」との回答が13病院、「看護補助者が不足している」との回答が16病院でした。また、いずれの医療機関からも、「診療報酬の大幅アップ」と「物価高騰に対する財政支援」を求める声が寄せられています。<br>　これらの結果を受けて記者会見を行い、山形新聞に記事が掲載されました。9月10日(火)には県への要請を実施しています。県としては、知事会等を通じて医療機関の実情に即した支援策を政府に要望していくとのことでした。</div> <div>　11月11日(月)～13日(水)にかけて、至誠堂総合病院の鶴田香看護師が能登地域への地域訪問支援に入りました。以下は、当時の様子を記した活動報告レポートです。</div><hr><div>&nbsp;<i class="fa fr-deletable"></i>&nbsp;</div><div>　初日は全焼した輪島朝市近くの漁師町へ。住宅が倒壊している。重機の入れない細道で全く片付いておらず、全壊、半壊で留守が多くとても静かだった。対照的に香箱ガニが解禁されたばかりで漁師の方々は活気が溢れており、「漁師は地震で失業していたんだ。復興の第一歩だ」と仰っていた。<br>　2～3日目は豪雨によって壊滅的な被害のあった、輪島市門前町浦上地区を訪問した。土石流が多発し、田畑に流木が散乱している。橋の崩壊で何度も迂回が必要だった。家の前の仮設住宅が床上浸水し、隣家は水害で全壊している70代の方からは「地震は何とか持ち堪えたが、水害で心が折れた」との言葉がこぼれた。全く進まない復興に、「能登は見捨てられたと感じる」との言葉が聞かれた。息子夫婦と暮らす70代の方は、ボランティアを頼まず自分たちで床上浸水の片付けをしたそうだ。「体力もあるし、水没した車や給湯器、室外機を新たに購入した。若い人と暮らしてるとボランティアを頼みづらいし、新しく買っても補助や援助はない」と仰っていた。100歳を超えるご高齢の方がいたお宅にも伺った。山奥で停電があったため避難所生活をしていたが、行動範囲が狭くなり、体調を崩して先日亡くなったとご家族よりお聞きした。「地震がなければ110歳まで生きるくらい、何でもやる元気なひとだったんだ」と仰っており、今は災害関連死の手続きをしているとの事だった。<br>　我慢強い県民性もあり、皆さんは文句を言わずに踏ん張っている。「他の大変な人に比べたらいいほうだ」と仰る。訪問した折には「話を聞いてくれてありがとう」と何度も言っていただいた。物理的な復興はもちろん、傾聴や共感などの心理的なケアの継続が今後も必要であると感じた。</div> <div>　7月25日(木)より降り続いた大雨で、庄内や最上地方で大きな被害が出ました。被災された皆様に心からお見舞い申し上げます。<br>　特に酒田市では、広範な地域で道路の冠水や床上・床下浸水などがみられました。山形県民医連の事業所でも25日の午前中に玄関前の駐車場が冠水し、患者様や職員の車が一時身動きできなくなるなどしました。通所サービスの利用者様を安全な場所でご家族様へ受け渡すなどの対応に追われると共に、クリニック内では下水が流れにくくなったため、一時的にポータブルトイレで対応しました。</div><div>　職員宅でも床上・床下浸水の被害が出ています。また、組合員や友の会員の方のところでも浸水被害が出ており、職員が片付け等の手伝いを行いました。</div><hr><div>　8月3日(土)には、県連事務局の職員が酒田市の災害ボランティアに参加しました。以下はその報告です。</div><div>　「当日は全体で130名の参加で5人ずつチームになって活動しました。自分は酒田市竹田（松山地区）の住宅地へ入り、1.2ｍ浸水したお宅で、がれきの撤去やトラックへの積み込み、泥の掻き出しを行いました。水没した小屋の中のものが散乱し、米袋が水に浸かって腐敗し、強烈な臭いが充満していました。<br>　自分が活動したところは冠水した地区でしたが、河川氾濫や土砂崩れによる被害に遭われた家屋は、重機による作業がまだ始まっていない地域が多いとのことです。今回伺ったお宅も、”月曜日に依頼して土曜日にやっと来てくれた”ということで、介入まで5日間を要したことになります。作業自体も30℃の炎天下の中、家主の方から依頼された半分程度しか進まず、後日別チームが入るよう申し送りをして終了となりました。今後も引き続きボランティア支援が必要だと感じています」</div>
